Records we need prior to the appointment

Please obtain the following records, if they have been done:

• MRI Brain and Abdomen reports AND images
• Renal ultrasound report
• Genetic testing results
• EKG and ECHO reports
• Cardiology notes
• Ophthalmology notes
• Neurology notes
• Nephrology/ renal notes
• Developmental testing, neuropsychological reports, IEP, school testing
• EEG reports
